The Agent Incident Register
A public, sourced database of AI agents going wrong, kept by EE Labs. Every incident has a permanent ID (EEAIR-YYYY-NNN), a confidence score computed from its sources alone, a severity, a cause, and a score against eight guardrails. An AI agent reviews the news every morning and drafts new entries in English and French; each entry says whether the agent added it.
