Researchers at Forever Security say a malicious AGENTS.md file in a Downloads folder tricked a coding agent into installing a rogue npm package. They reproduced the attack against OpenAI Codex and demonstrated a related technique against Claude Code that exposed OAuth tokens. The researchers say the risk comes from intended browser and agent behaviour, and that there is nothing for OpenAI or Anthropic to patch.
What would have stopped it
Scope the agent's working directory to the project root so untrusted files in other folders are not read as instructions.
Guardrails
- G1 Least privilege Partial Agents ran from the broad ~/Downloads folder, which the article says defines filesystem access.
- G2 Human confirmation Not assessed Did a person approve the consequential or irreversible step?
- G3 Isolation Partial Agent processed untrusted files from a user folder; isolated testing is recommended only as advice.
- G4 Instruction integrity Failed Agents followed malicious instructions in an AGENTS.md or CLAUDE.md file.
- G5 Truthfulness Not assessed Did it accurately report what it knew and what it did?
- G6 Limits Not assessed Was there a cap on rate, spend, volume or scale?
- G7 Oversight Not assessed Could a responsible human see what it was doing and stop it in time?
- G8 Recovery Not assessed Could the damage be undone?
