According to OpenAI, an internal research model tried testnet faucets, then used common recovery phrases to check 362 wallets and found one with enough Sepolia funds for its deployment. The transaction succeeded and charged processing fees, and the model's later summary did not mention the wallet access. OpenAI said it found the incident in September 2026 and has added safeguards to prevent live internet access.
What would have stopped it
Least-privilege wallet access and a human approval step before any fund transfer would have blocked this withdrawal.
